APPEAL FORMS

Academic Appeal Form: Academic Decisions

This form is utilized to appeal an Academic Decision with the Perkins College of Education.

Please make sure to read the included policy on disputing Academic Decisions, to ensure that appropriate time frames are met.

Academic Appeal Form: Academic Suspension

**The deadline for submission of an Academic Suspension Appeal for the College of Education is 7 "business days" before the first class day of the semester being requested for reinstatement.

--> All suspension appeals for return during the Fall 2018 semester are due to be submitted by 5:00p.m. on Friday, August 17, 2018. Appeals will be reviewed the week of Aug 20th. Decisions will be sent to the students mySFA @jacks email account only.

This form is to be utilized by Perkins College of Education Undergraduate Students who are on Academic Suspension and wish to be considered for reinstatement during the Fall or Spring semester.

If placed on Probation, Continued Probation or Suspension, it is your responsibility to communicate with your Student Success Advisor.

Undergraduate Student Forms and Applications

Degree Plan Request

Spring 2018 requests were due by 5pm on Thursday, March 29, 2018. We will begin accepting requests again on Monday, June 4, 2018.

When instructed by delegated Academic Advisor, student will submit this form to request a degree plan. Typically, students may request a degree plan beginning at 45 completed hours.

Late Add or Course Reinstatement Request

**IMPORTANT** Late adds or course reinstatements must be completed by 5:00 p.m. on the University's 20th official class day (Fall/Spring) or 15th official class day (Summer).

After obtaining signatures, the Dean's Office will fax a copy of this form to the SFA Business Office and Registrar's Office. The student must then immediately contact the Business Office at (936)468-6960 for required payment amount and instructions. After payment is made, the Business Office will contact the Registrar's Office to complete the enrollment.

Senior Year Overlap Form

In this program, the student may be permitted to take up to 12 hours of graduate level courses to apply toward a graduate degree at SFA.

Report of Academic Dishonesty

This form is to be completed by the Professor when Academic Dishonesty occurs. The completed form with all documentation is to be sent to the appropriate dean of the college for the students' program of study.
