APPEAL FORMS

Academic Appeal Form: Academic Decisions

This form is utilized to appeal an Academic Decision (disputes with a professor related to a decision or a grade) with the Perkins College of Education. Students must appeal at each level and acquire a resolved decision before moving on to the next noted level of appeal.

Please make sure to read the included policy on disputing Academic Decisions, to ensure that appropriate procedures are followed and required time frames are met.

Academic Probation, Suspension, and Reinstatement for Undergraduates

If placed on Probation, Continued Probation or Suspension, it is the students responsibility to communicate with their Academic Advisor immediately upon receipt of their notification letter/email. Students are required to follow the policy as stated in their letter and posted on the link attached.

Undergraduate Suspension Appeal Form *COLLEGE of EDUCATION ONLY*

*The suspension appeal form will be made available during submission deadlines. If needing additional assistance, please email coestudentservices@sfasu.edu.

This appeal form is for undergraduates in the college of Education, who had a documented extenuating circumstance during their most recent semester and would like to be considered for return during the next upcoming (Fall or Spring) semester.

For return during the Fall 2019 semester (no longer accepting Fall appeals): Students must submit their appeal form, letter of appeal and supporting documentation to Dr. Stacy Hendricks, Associate Dean, by 5:00p.m. on Monday, August 19, 2019. This is a firm deadline. Submissions can be via email to coestudentservices@sfasu.edu or brought to the McKibben 118 front desk. Once decided upon, the outcome of the appeal will be emailed to the mySFA email of the student.

Please note: This form is NOT used to appeal a financial aid decision.

Undergraduate Student Forms and Applications

Degree Plan Request

Upon completing 30 hours, students must request a degree plan for their desired program of completion.

Late Add or Course Reinstatement Request

**IMPORTANT** Late adds or course reinstatements must be completed by 5:00 p.m. on the University's 20th official class day (Fall/Spring) or 15th official class day (Summer).

After obtaining signatures, the Dean's Office will fax a copy of this form to the SFA Business Office and Registrar's Office. The student must then immediately contact the Business Office at (936)468-6960 for required payment amount and instructions. After payment is made, the Business Office will contact the Registrar's Office to complete the enrollment.

Senior Year Overlap Form

In this program, the student may be permitted to take up to 12 hours of graduate level courses to apply toward a graduate degree at SFA.

FACULTY & STAFF FORMS

Report of Academic Dishonesty

This form is to be completed by the Professor when Academic Dishonesty occurs. The completed form with all documentation is to be sent to the appropriate dean of the college for the students' program of study.

WH Grade Report

This for must be completed by PCOE faculty prior to entering the grade of WH for students. Once signed by all those required, the form needs to be emailed to coestudentservices@sfasu.edu.
