Academic Appeal Form

This form is to be utilized by Students who are on Academic Probation or Suspension and wish to be considered for reinstatement during the Fall or Spring semester. You must communicate your Academic Status with you Academic Advisor each semester.

Aide Exemption

This form is to be submitted if you believe you have met the requirements to be exempt from Student Teaching. You must submit your application with proof of receiving Educational Aide funds and proof of having successfully used delegated funds (and having successfully completed) for such noted semester.

Application for Admission to Educator Certification: Prior to Fall 2013

This application is to be completed for Admission to the Educator Certification program if your first time SFA registration was prior to the registered Fall 2013 semester. Please review all admission requirements prior to submitting your application.

Application for Admission to Educator Certification: After Fall 2013

This application is to be completed for Admission to the Educator Certification program if your first time SFA registration was during or after Fall 2013. Please review all admission requirements prior to submitting your application.

Degree Plan Request

When instructed by delegated Academic Advisor, student will submit this form to request a degree plan. You may request a degree plan beginning at 45 completed hours, and must have one on file by completion of 60 hours.

Degree Plan Request for ONLINE Completers

When instructed by delegated Academic Advisor, online completers will submit this form to request a degree plan. You may request a degree plan beginning at 45 completed hours, and must have one on file by completion of 60 hours.

Graduate Tracking Forms

This form is to be submitted to the Graduate Program Coordinator, or a Certification based Graduate Level Program.
