Student responsibilities include, but may not be limited to, the following:
Receive admission to the University.
Meet with the Director of Disability Services prior to initial semester in which accommodations will be requested to self-identify as a qualified student with a disability.
Make specific request for reasonable accommodations in a timely manner.
Provide documentation as specified in SFA guidelines.
Participate in the accommodation process and understand that the process is intended to be interactive and collaborative.
Initiate appointment with Disability Services each semester in which accommodations are requested.
Initiate conference with each professor who receives a Special Accommodation Request to insure mutual understanding of how accommodations will be provided.
Contact Disability Services if questions or difficulties related to the provision of accommodations arise that cannot be resolved between the student and faculty/staff.
Abide by the Code of Conduct and other regulations applicable to all students enrolled at SFA.
Be familiar with the campus evacuation procedures.
