Immunization and Test Requirements
Immunization Records
Stephen F. Austin State University strongly advises all students to submit immunization records to the SFA Student Health Services.

The SFA Health Service’s Immunization Record Form can be filled out, or the vaccination record can be submitted in any format, please provided basic identification information: student’s full name, date of birth, contact information, and campus identification number (if assigned). Usually, you can obtain a copy of your immunization records from your high school.
- Diptheria/Tetanus/Pertussis (Tdap)
- Polio (IPV) Inactivated Poliovirus Vaccine
- Measles (Rubeola)/Mumps/Rubella (MMR)
- Varicella Vaccine (Chicken Pox)
- Hepatitis A Vaccine (HepA)
- Hepatitis B Vaccine (HepB)
- Human Papillomavirus Vaccine (HPV)
Meningitis Conjugate Vaccine (MCV): The State of Texas has passed a new law (HR 4189) that requires all first-time freshman and transfer students who live in campus residence halls to have a vaccination against bacterial meningitis. Some degree plans require submission of immunization records (or proof of immunity) of specific vaccinations to enroll in their program. See the admission requirements of the department in which you wish to major.
