The SFA Health Service’s Immunization Record Form can be filled out, or the vaccination record can be submitted in any format, please provided basic identification information: student’s full name, date of birth, contact information, and campus identification number (if assigned). Usually, you can obtain a copy of your immunization records from your high school.
Some degree plans require submission of immunization records (or proof of immunity) of specific vaccinations to enroll in their program. See the admission requirements of the department in which you wish to major.
Tuberculin (TB) skin testing is offered year around, at no cost, for any currently enrolled student. The test can be performed annually if you have never had a positive TB test. You must be able to return to the clinic for the test to be ‘read’ by a nurse within 48 to 72 hours after receiving the antigen. For this reason, we can only administer the skin tests on Monday, Tuesday, or Fridays. If you do not return to the clinic within this period of time, it will be necessary to repeat the test – but only after a period of at least 1 month has elapsed. A $10.00 charge will be assessed for a repeat test.
