Tips for a Great Medical Visit

As a patient, you have a right to receive quality health care. You also have a responsibility to be prepared for your medical visit and provide necessary accurate information to your medical provider.

Medical History:

Know your medical history. This means previous diagnosis, medical conditions, surgeries, immunizations and medications. It is important to also know the medical history of your immediate biological family. Before your fist visit, you can save time if you print out the Medical History Form.  Bring the completed form and Campus ID to your scheduled appointment.

Transferring Medical Records:

Have pertinent medical information sent to SFA Student Health Services by providing your current medical provider(s) with a completed Medical Release Form.

Medications:

Know the names of the medications you are taking. Bring the bottles of any current medications to your visit.

Allergies:
Know your allergies to medications, foods, and other materials, such as latex. Knowing what you are allergic to and what kind of reaction you have had will help your provider prescribe the best medicine for your needs.

Participate: Ask questions about your diagnosis, treatment plan, and recovery time. We encourage you to discuss your questions or concerns with your medical provider.

Questions: You do not need health insurance to be seen at the Health Services clinic. But it is helpful to know about your insurance coverage in case referrals or outside tests are needed. If you do not have health insurance, Stephen F. Austin State University endorses an optional Student Insurance Plan for eligible students.

Prepare:
Being prepared will help you get what you want out of your visit. This is your time, so use it wisely! You can write your questions down if that will help you remember what you want to ask. Prioritize your questions and concerns so that your provider can focus on what is most important to you.
