Faculty-Led-Program Application
You are required to complete and sign all forms. After you have completed the application package, submit them to the Office of International Programs along with the application fee payment slip. Space is limited in all programs, so early registration is strongly recommended. For more information, contact:

- Waiver of Liability *
- Study Abroad Acceptance, Release and Waiver *
- Authorization to Discuss/release Confidential Student Academic Information
- Health Insurance Enrollment Form *
- Health Information Form *
- Medical Treatment Permission Form *
- Two copies of your Passport – First page and inside cover.
Reminder
- If you are applying for the OIP scholarship, you need to submit an unofficial copy of your transcript (this can be printed from mySFA).
- If you don't you have your passport by the time you are applying to the study abroad program, please submit two copies as soon as you get it.
- Our office will not be able to make any passport copies.
