Change in Department Head

Any time there is a change in department head, the out-going department head (interim or permanent) must complete a PHYSICAL inventory accounting for all inventory for which he/she is responsible. Every piece of equipment must be accounted for either by physically locating it or by requesting removal by one of the appropriate and documented methods. The out-going physical inventory must be completed and a Change in Department Head Certification of Physical Inventory submitted to the Property Manager within 30 days prior to the last day of service (interim or permanent).

The incoming department head (interim or permanent) must also complete a PHYSICAL inventory accepting responsibility for all equipment relinquished by the out-going department head. Every piece of equipment must be accounted for either by physically locating it or by requesting removal by one of the appropriate and documented methods. The in-coming physical inventory must be completed and a Change in Department Head Certification of Physical Inventory submitted to the Property Manager within the first 30 days of service in the department head position (interim or permanent).

A current inventory list must be requested from the Property Manager. The Change in Department Head Certification of Physical Inventory form will be provided with the current properly list or may be printed from the ITS FormServer.
