Graduate Program Forms for the Perkins College of Education

Academic Probation/Suspension

ACADEMIC PROBATION HOLDS:

  • PCOE Graduate Studies Coordinator will run report and notify Program Coordinators of
    students and their standings.
  • PCOE Graduate Studies Coordinator will send letters and emails to students notifying them of their standing.
  • Program Coordinators need to email coegrad@sfasu.edu to request Probation Hold be removed after discussing standing and degree requirements with student.
  • If approved for continuation and cleared for registration, the PCOE Graduate Studies Coordinator will remove the probation hold.

ACADEMIC SUSPENSION HOLDS:

  • PCOE Graduate Studies Coordinator will run report and notify Program Coordinators of
    students and their standings.
  • PCOE Graduate Studies Coordinator will send letters and emails to students notifying them of their standing.
  • Program Coordinator will determine continuation eligibility for student, then email coegrad@sfasu.edu if requesting continuation.
  • If it is determined that the student is eligible for program continuation, the PCOE Graduate Studies Coordinator will contact the Dean of Research and Graduate Studies.
  • Upon Approval or Denial, the Program Coordinator will be contacted to relay final decision to the student.
  • If approved for continuation and cleared for registration, the PCOE Graduate Studies Coordinator will remove the suspension hold.

Admission to Candidacy: Ed.D. and Ph.D. Only

Processing Procedure:

  • Form is to be completed by Doctoral Program Coordinator.
  • Academic Unit Head will review and sign where indicated, if approved.
  • Form will be scanned and emailed to coegrad@sfasu.edu.
  • PCOE Graduate Studies Coordinator will acquire additional approvals required.
  • A copy will be kept in student record in the Student Services and Advising Center.

Graduate Degree Plan

Processing Procedure:

*This form is to be completed upon candidates successful admission to the program. Form can be downloaded for applicable program drop-down options.

  • Form is to be completed by Program Coordinator.
  • Academic Unit Head will review and sign where indicated, if approved.
  • Degree Plan will be scanned and emailed to coegrad@sfasu.edu.
  • PCOE Graduate Studies Coordinator will review and approve degree plan.
  • A final electronic copy will be sent via email to Academic Unit Head, Program Coordinator and Administrative Assistant.
  • Program Coordinator will provide student a final copy.
  • A copy will be kept in student record in the Student Services and Advising Center.

Late Add/Course Reinstatement Request

**IMPORTANT** Late adds or course reinstatements must be completed by 5:00 p.m. on the University's 20th official class day (Fall/Spring) or 15th official class day (Summer).

The student must initiate this form and route through all required approvals.

After obtaining signatures, the Dean's Office will fax a copy of this form to the SFA Business Office. The student must then immediately contact the Business Office at (936)468-6960 for required payment amount and instructions. After payment is made, the Business Office will contact the Registrar's Office to complete the enrollment.

Notice of Admission

Processing Procedure:

  • Form is generated by Office of Research and Graduate Studies and provided to the Program Coordinator.
  • Program Coordinator will review, approve or deny, and sign where indicated.
  • Form will be scanned and emailed to coegrad@sfasu.edu.
  • PCOE Graduate Studies Coordinator will review and forward to Office of Research and Graduate Studies.
  • A copy will be kept in student record in the Student Services and Advising Center.
  • Final processing occurs in the Office of Research and Graduate Studies.

Overlap Recommendation

Processing Procedure:

  • Form is to be completed and approved by Academic Unit Head.
  • Form will be scanned and emailed to coegrad@sfasu.edu.
  • PCOE Graduate Studies Coordinator will forward to Office of Research and Graduate Studies.
  • A copy will be kept in student record in the Student Services and Advising Center.
  • Office of Research and Graduate Studies Dean provides final approval.

Petition to Accept Out-of-Date Courses

Processing Procedure:

  • Form will be initiated by the Student.
  • Program Coordinator/Instructor will provide course information and validation.
  • Academic Unit Head will review and sign where indicated, if approved.
  • Form and supporting documentation will be scanned and emailed to coegrad@sfasu.edu.
  • PCOE Graduate Studies Coordinator will acquire additional approvals required, and forward to Office of Research and Graduate Studies.
  • A copy will be kept in student record in the Student Services and Advising Center.
  • Office of Research and Graduate Studies Dean provides final approval.

Petition to Change Graduate Degree Plan

Processing Procedure:

  • Form is to be completed by Program Coordinator.
  • Academic Unit Head will review and sign where indicated, if approved.
  • Form will be scanned and emailed to coegrad@sfasu.edu.
  • Degree plan change request(s) will be reviewed and processed in the Student Services and Advising Center. A final copy will be kept in student record.
