Operational Changes:

Health Services remains available for enrolled or registered students with questions or in need of medical assistance during our regular hours, 8 a.m. to 5 p.m., Monday through Friday.  Due to the current COVID-19 pandemic, and in being in compliance with SFA policies regarding social distancing, we are temporarily implementing changes in our operational process:

  • You are required to call the clinic at 936.468.4008 and schedule an appointment.
  • This includes clinic services such as, but not limited to, TB testing, urinary drug screen, administration of a vaccine or allergy injections.
  • When you arrive at the clinic, please call 936.468.4008 to let the staff know you have arrived for your appointment.
  • Wait at the door and someone will be with you as soon as possible
  • Practitioners / nurses are speaking with students via phone or by ZOOM.
  • At this time, we will not be accepting walk-ins.
  • Face coverings are required for all patients.

New Patient Forms

If you have never been to the Health Clinic previously, please print, complete and bring the following forms with you to your scheduled appointment. We recommend that you arrive 15 minutes early to take care of registration and paperwork. Remember to bring your valid SFA ID card. (All forms are PDFs.)

SFA’s health clinic offers a full range of medical services, from checkups and preventive care to immunizations to contraceptives and STI testing. Our staff of experienced, licensed physician, physician assistant, nurse practitioner, registered nurses and certified lab technicians will help ensure that your time at SFA is happy and healthy.

Appointments are required for most visits and usually available same-day if you call early (8 to 8:30 a.m.) or next day. Urgent work-in appointments are available for minor injuries and asthmatic or allergic reactions.


Students registered or enrolled at SFA may make appointments for the following services:

  • Examination, diagnosis and treatment for medical illnesses
  • Wound care, including minor surgery and suture removal
  • Physicals
  • Tuberculosis (TB) testing
  • Urinary Tract Infection (UTI) screening and treatment
  • Elective Lab Testing
    • Drug Screens
    • Sexually Transmitted Infection (STI) testing and treatment
      Free biannual blood test screening for HIV and Syphilis
      Free quarterly urine test screening for Chlamydia and Gonorrhea
  • Routine gynecological services and contraceptive counseling
  • Immunizations
  • Allergy management
  • Mental health services (in partnership with SFA Counseling Services)
  • Full service Laboratory

Making an appointment

Appointments are available Monday through Friday from 8:30 to 11:30 a.m. and 1 to 4:30 p.m. To make an appointment, call the health clinic at 936.468.4008.

  • Most appointments can be made 1 day in advance. For gynecological visits, we recommend that you make an appointment a week in advance.
  • If you need to cancel or reschedule your appointment, please notify the health clinic at least 24 hours ahead of time.
  • Students who arrive more than 10 minutes late will need to reschedule their appointments.

Insurance and payments

As an SFA student, you have access to most health services free of charge (we require payment for some immunizations and medical tests). Your student services fee assists in paying for some of the cost of your medical care while you’re registered and enrolled at SFA.

Although most health services are available free of charge, the health clinic does accept many insurance plans. This helps us keep costs down and enables us to provide high-quality care to all Lumberjacks. If you have health insurance, you should bring your health insurance card to every appointment at the health clinic.

While your care at the health clinic is available whether or not you have health insurance, injuries and after-hours illness are not uncommon while in college.  We strongly encourage you to obtain health insurance coverage, as emergency room visits, urgent care visits, and hospital stays can be extremely costly.


The health clinic provides a variety of immunizations, including:

  • Diphtheria/Tetanus/Pertussis
  • Hepatitis B Vaccine 
  • Human papillomavirus vaccine (HPV)
  • Bacterial Meningitis vaccine (By law, all students are required to have this vaccine at least 10 days prior to enrollment.)
  • Flu vaccine
  • Tuberculin (TB)
    TB skin testing is offered year-round for any currently enrolled student. You must be able to return to the clinic for the test to be read by a nurse within 48 to 72 hours after receiving the antigen. For this reason, we can only administer the skin tests on Mondays, Tuesdays, or Fridays. If you do not return to the clinic within this period of time, it will be necessary to repeat the test after a period of at least one month has elapsed. A $12 charge will be assessed for each test given.

Do you want your immunizations records stored in one place? ImmTrac2 is a voluntary immunization registry service provided by the Texas Department of State Health Services. By utilizing this service you can safely and confidentially store your immunizations in one location.

Medical records

  • If you need to send your medical records TO SFA from another provider or facility, you’ll need to fill out the Medical Release to SFA form. 
  • If you need to send your medical records FROM SFA to another provider or facility, please fill out the Medical Release from SFA form.
