The Verification Department of the Office of the Registrar makes changes to student's account regarding names, addresses, and Social Security numbers.

Name Change

Requests for name changes must be made in person or by mailing or faxing the online Name Change form.

The following documentation is required for name changes:

  1. Social Security Number
  2. Previous Name
  3. Photocopy of a marriage license, court order, or dissolution decree reflecting the new name in full;

    or

    For non-U.S. citizens: Current passport or official proof of identity, certified by U.S. embassy abroad or by the appropriate foreign embassy in the U.S.

Update Social Security Number

If for any reason you should need to update your Social Security Number with Stephen F. Austin State University, you will need to bring a copy of your SSN card to the SFA Registrar's office to have your student record updated.
