Credit Card Convenience Fee: According to SFA Student Business Services, the fee is 3.0% with a $3 minimum, which is non-refundable.

Camp Liability Insurance: Rate dependent on day camp or overnight camp, number of minors, staff and days. You can request breakdown of rates at youthprograms@sfasu.edu. 

Insurance is required for youth programs that are practicing any sports, staying overnight, or being left under the custodial care of SFA.

Steps to follow through the University of Texas System:

1. You may access the Online Insurance Form in the ORM Camp Portal with your institution credentials. Click "New Camp Application," then choose the type of camp you will be hosting: nonsport, sport, or onlines, and the respective application will appear. Note: the form will not let you submit it without answering all questions.
    
2. A copy of your submission is sent to the UT System ORM Camp Policy Coordinator for further processing.
    
3. Southwest Special Risk Insurance will send documents to the camp contact on the application from Tammy Westbrook (tammy_westbrook@outlook.com). 
    
4. Once camp is finished, ORMinfo@utsystem.edu will send Program Directors an audit form that will need to be filled out with the actual number of students that attended the youth program. This must be completed within 72 hours.
    
5. Youth Program Directors must complete steps for payment after the completion of the youth program. Failure to do so may result in the non-acceptance of future camp applications.

Report any incidents involving minors to the following link:

Texas law requires anyone with knowledge of suspected child abuse or neglect to immediately report it to the Department of Family and Protective Services (DFPS). 

Reports can be made at any time by phone (1-800-252-5400) or online at the Texas Abuse Hotline. The hotline and website are available 24 hours a day, seven (7) days a week. 

The university's policy prohibits retaliation against good faith reporting of suspected abuse or neglect.
