Academic integrity violations

Drug/Alcohol Use Report

File police report
Visit the University Police building to file a report. Anonymous police reports may be filed through the Rave guardian mobile app.
Title IX misconduct
If you have directly experienced sex- or gender-based misconduct on campus or during a university-sanctioned event, self-report through the sexual misconduct reporting form. Ref: Office of Title IX
University employees have the duty to report an incident when they receive information about an alleged incident. Ref: Mandatory reporting, Office of Title IX
Duty to report child abuse or neglect
Texas law requires anyone who has reason to believe that a child has been abused or neglected to report the suspected abuse or neglect. Ref: Duty to report
Complaint resolution
SFA strives to ensure all students, employees and third parties are treated in a manner in accordance with university policies. Should an instance arise in which you believe that a department or an individual has not acted in accordance with this stated goal, you should first:
- seek to resolve the issue informally with the respective individuals involved, or their supervisor, following appropriate escalation points:
- Staff member → Department supervisor → Division leader
- Professor → Department chair/director → College associate dean → College dean → Associate provost → Provost
If these informal procedures prove unsatisfactory, you may file a formal grievance to seek resolution.
Formal grievance
A formal grievance is a written student complaint that alleges
- there has been a violation or disparate application of a university policy or written procedure
- and the grievance has been unsuccessfully resolved through informal resolution strategies.
Students may file a formal written complaint online using their mySFA credentials.
Employees and third parties should follow the steps outlined for Institutional Complaints.
External options
SFA is accredited by the Southern Association of Colleges and Schools, whose complaint process may be found on their SACSCOC policy "Complaints Against SACSCOC or Its Accredited Institutions".
Students not satisfied with the outcome of the institutional appeals process may file a complaint under the appropriate authorizing agency:
- In-state students: Texas Higher Education Coordinating Board student complaint process.
- Out-of-state distance education students: State Authorization Reciprocity Agreement student complaint process.
