Forms and Resources

These advising forms are specifically created for Rusche College of Business students. To access these forms, you must use your mySFA log in credentials.

• Course petition request for credit that may apply to your degree plan.
  • The Office of the Registrar must have your transcript on file prior to petition submission.
  • Attach proper documentation with your petition request.
• Course overload request to take more than the maximum allowed semester credit hours per semester.
  • View the registrar's office information on maximum hours.
• Major/minor change request to declare your business major, add a business minor or change to a business major.
• Suspension appeal should be completed after grades are posted for the semester and you've scheduled a meeting with your academic advisor.

Late add requests

If you're requesting to be added to a class after the first class day, complete the late add or course reinstatement request.
