These advising forms are specifically created for Rusche College of Business students. To access these forms, you must use your mySFA log in credentials.
- Course petition request for credit that may apply to your degree plan.
- The Office of the Registrar must have your transcript on file prior to petition submission.
- Attach proper documentation with your petition request.
- Course overload request to take more than the maximum allowed semester credit hours per semester.
- View the registrar's office information on maximum hours.
- Major/minor change request to declare your business major, add a business minor or change to a business major.
- Suspension appeal should be completed after grades are posted for the semester and you've scheduled a meeting with your academic advisor.

Late add requests
If you're requesting to be added to a class after the first class day, complete the late add or course reinstatement request.
